All of the forms are interactive, so you can type your information on the form. Then please print the forms and bring them into one of our 10 branches in order to complete the switch.

To start, decide if you want to open a personal account or a business account. Then fill out the Personal Account Information Sheet or the Business Account Information Sheet respectively. If you currently have direct deposit, then fill out either the Direct Deposit Change Form or a Government Direct Deposit Sign-up Form. If you operate your finances with automatic payment, then fill out the Automatic Payment Change Form. Lastly, upon reviewing the Account Transfer Checklist, fill out the Account Closure Request Form. Just print or copy as many forms as you need and bring them into the Bank of Jackson Hole to complete the opening of your new BOJH account. Remember to bring two forms of ID and a utility bill.
